Referrals
We require 48 business hours to complete referrals. It is the policy of this office NOT to post date referrals as specified by the insurance companies.
There will be a charge of $10-$40 for the completion of forms, depending on length and complexity. We require 5 business days for the completion of these forms.
Medical forms
There will be a charge of $20.00 for missed appointments without cancellation.
